What We Do

At Infinite Possibilities Care Connections (IPCC), we are committed to empowering individuals with developmental and intellectual disabilities by providing comprehensive, person-centered support coordination through our experienced Coordinators of Community Services (CCS).

Our targeted case management services are designed to enhance the quality of life for the people we serve by helping them access necessary resources, services, and opportunities.

  • We begin with a comprehensive needs assessment to understand the individual strengths, challenges, and preferences. Based on this, our CCS develops a person-centered plan (PCP) that reflects the individual aspirations and outlines the services and supports necessary to achieve them.

  • IPCC’s CCS acts as the primary point of contact, coordinating access to a wide range of services, including healthcare, housing, education, vocational programs, and other community resources. We work collaboratively with service providers, ensuring seamless communication and efficient service delivery.

  • Our team provides continuous oversight and regular follow-up to ensure that services are being delivered as planned. We regularly review each individual’s progress and make adjustments to their person-centered Plan (PCP) as needed to ensure that goals are met and any changes in needs are addressed.

  • At IPCC, we believe in empowering individuals to advocate for themselves. Our CCS team supports individuals in navigating complex systems, understanding their rights, and advocating for the services and supports they need. We ensure that their voices are heard and that they have access to the resources necessary to live fulfilling lives.
